A copy of forms such as health records or proof of identity should be placed in the extended day area or copied electronically to ensure it is accessible when the school office would be closed.

Comments:
An unannounced monitoring visit was conducted. Ten children were present with five caregivers. The children had field day and an early release so numbers were very low. Snack provided today was chips and salsa and juice. No medication was present for children in the extended day licensed program (ages 5-12). Two staff do have MAT and the MAT diabetic module for a student in 8th grade who occasionally attends the program. Emergency supplies are up to date. Children were observed weaving, free play and hand washing prior to snack. Preschool program is closed for the school year. The provider's responses for the "Description Of Action To Be Taken" were not received as of 6/17/2015 and will not appear on the Violation Notice. Please call if you need any assistance. Description: Based on staff statement, records for children are not always accessible during hours when the office is closed. Evidence: Both physical and immunization records as well as proof of identity are kept in the clinic/office which was locked during the inspection as the school had released early that day. Neither were accessible to the inspector for review.

Standard #: 22VAC40-185-70-A
Description: Based on record review, it was determined that a staff member record was incomplete. Evidence: Staff #1 had only one of two required references prior to employment.

Standard #: 22VAC40-185-550-D
Description: Based on the fire drill log, it was determined that an evacuation drill had not been conducted each month. Evidence: The drill record did not reflect an evacuation drill for the months of February and March 2015. Additionally, drills were only conducted during the school day. Drills should be conducted during all hours of operation (both before school and after school) to ensure children are familiar with exiting from the areas of the building where they might be during the extended day program.

Description: Based on record review, it was determined that a staff member was missing background checks within 30 days of employment. Evidence: Staff #1 was missing both a Criminal Record Check and a Child Protective Search. The date of hire was 8/2014.
