Comments:
An unannounced monitoring inspection was conducted 5/7/2024 from 9:45am-10:55am. There were 8 children (16 points) directly supervised by 2 caregivers. The home, 2 caregiver records, 3 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed doing origami craft activity, sing along and practicing names of body parts (head shoulder knees and toes). There was an assortment of age appropriate materials for children. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-800-90-A
Description: Based on record review and interview, the provider did not obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations, before child attended.
Evidence:
1. Child #3 first day or attendance 11/29/2023, immunization documentation received after 12/11/2023 immunizations received.
2. Provider acknowledged they were aware immunization documentation was received after child's first day.

Plan of Correction: Provider will require parents provide documentation of adequate immunizations prior to/on child's first day in the home day care.
