22VAC40-191 Background Checks (22VAC40-191)
66 Children were in care during the inspection. The age range of the children was four months to nine years old. There was a special event today the children were playing in a bouncy house. Five children's files were reviewed. Three staff are current in MAT. Inspection time 1:00pm-4:00pm
Standard #: 22VAC40-185-70-A Description: Based on review of three new staff records not all were complete. Evidence: Staff A and B did not have written documentation of work experience and education to meet lead teacher qualifications. Plan of Correction: I will get a copy of their transcripts and previous work experience.
Standard #: 22VAC40-185-450-A Description: Based on observation not all children had linens on the rest cots. Evidence: In the Owl room there were two children resting without a top lien and two children resting without either a top or bottom lien. Plan of Correction: We will buy extra liens .
Standard #: 22VAC40-185-510-F Description: Based on the review of medications not all had labels attached. Evidence: The medication for child A did not have the original container with the direction label attached. Plan of Correction: I will have the parents bring in the original container
Standard #: 22VAC40-185-510-J Description: Based on review of medications not all were kept in a locked place. Evidence: In the owls room there was medications stored in a unlocked cabinet accessible to the children and in the school age room there was a staff's purse stored on a shelf accessible to the children. The staff stated there was a emergency inhaler in the purse. Plan of Correction: All medication were moved to a locking cabinet.
Standard #: 22VAC40-191-60-C-2 Description: Based on review of staff records not all had current background checks with in thirty days of employment. Evidence: Staff A with start date of 5/13/2015 did not have documentation of a criminal history search. Plan of Correction: The staff will not continue working until the criminal history check has been returned.
