Comments:
An unannounced inspection was conducted beginning at 7:45am regarding a complaint received in the licensing office. Morning ratios in all classrooms met minimum requirements with staff to children. There were 39 children present (with more children and staff arriving after the head counts were completed) with 9 staff. Children were playing with games and toys and painting pictures. A mandated inspection was also conducted during this visit. There were no observed violations with the physical plant or supervision of children. Repeat violations were found in record keeping. This inspection ended at 9:15am. An exit interview was conducted on-site at this time.

Complaint related: No
Description: Based on record review, two staff records (#1 and 2) have no documentation of a TB test submitted within 21 days of their start date. Evidence: Staff #1 has a start date of 3/3/15. Staff #2 has a start date of 2/27/15. No TB tests are in their records.

Plan of Correction: Staff #1 is on extended leave and we will request a TB test when they return. Staff #2 was also out on leave and we will request their TB test as well when they return.

Complaint related: No
Description: Based on record review, three children's records (#1,6 and 7) are missing complete required documentation in their files. Evidence: Child record #1 has a start date of 5/1/13 and has no documentation of a physical examination obtained within 30 days of their start date. Children's records #1,6 and 7 do not have documentation that parents have annually reviewed and updated any changes in the children's files. The last updates are dated as follows: 5/1/13 for child #1, 3/3/13 for child #6 and 1/1/13 for child #7.

Plan of Correction: With summer camp starting we did not get a chance to update older files. We will start updating all files with parents this week.

Description: Based on discussion a current annual fire inspection has not been conducted. Evidence: No previous fire inspection can be located.

Plan of Correction: We have the fire inspection scheduled for September, 2015. We we request annual fire inspections with the county.
