Technical Assistance:
Viewing the child's proof of identity is not necessary when the child attends a public school in Virginia and the center assumes responsibility for the child directly from the school (i.e., after school program) or the center transfers responsibility of the child directly to the school (i.e., before school program).

Comments:
An unannounced monitoring inspection was conducted on July 24, 2015. The children were observed transitioning from outdoor play, journaling, during center time, watching children's programming, and having lunch. The licensing inspector reviewed the following: indoor and outdoor areas, emergency supplies and procedures, nutrition, fire and health inspections, required postings and supervision. Three children's records were reviewed during this inspection. One medication was reviewed. At arrival to the facility, there were 18 school age children present with two staff members.

Violations:
Standard #: 22VAC40-185-510-C
Description: Based on observation and inspection of the facility, the licensee did not ensure that the parent's authorization for medication, expires and has to be renewed after 10 work days. Long-term prescription drug use and over-the-counter medication may be allowed with written authorization from the child's physician and parent. Evidence: 1. During the inspection conducted on July 24, 2015, the licensing inspector observed a medication authorization form with the effective date of 6/17/15. The authorization form was effective until 6/29/15. The parent had not renewed the authorization and there was not a long term authorization from the physician. 2. Documentation in the medication administration log indicated that the medication had been given to the child beyond the effective date of the authorization on the following dates: 7/1/15; 7/6/15; 7/8-7/10/15; 7/13/15, 7/15-7/17/15 and 7/20-7/22/15.

Plan of Correction: We will have the parent to update the authorization.
