An unannounced monitoring inspection was conducted on July 21, 2015. The director was present for the inspection. In Room # 1, there were 6 children, ages 2 to 3 years, with 1 staff. The children were doing art and playing in housekeeping and with a train. Room # 2 included 6 children, ages 3 years, with 1 staff. The children were dancing with their teacher. Room # 3 and # 4 included 10 children, ages 6 to 7 years, with 1 staff. The children were dancing and playing dress-up. In Room # 5 there were 9 children, age 4 years, with 1 staff. The children had just returned from having water play and were changing their clothes. Room# 6 included 7 children, ages 4 to 5 years, with 1 staff. The children were coloring and using the computer. Room # 7 included 8 children, ages 4 to 5 years, with 1 staff. The children were playing with blocks and housekeeping, using the computer, and painting. Lunch was served during the inspection and the menu for the day was tacos with cheese, lettuce, and tomatoes, oranges, carrots, and milk.

Standard #: 22VAC40-185-330-B Description: Based on observation, the center did not ensure that the resilient surfacing was the required depth in a fall zone. Evidence: The pea gravel used for resilient surface in the fall zones for the school age swing set measured 3 to 4 inches in depth. Plan of Correction: The maintenance person will rake and redistribute pea gravel to the correct depth.
