An unannounced monitoring inspection was conducted on 05/20/2024. There were 6 children, ages 2 years- 4 years, totaling 12 points and 1 caregiver present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, preventing the spread of disease, emergencies and nutrition. A total of 5 children?s records and 2 caregiver records were reviewed. The children were observed having breakfast, in free choice play, participating in learning activities, and watching educational videos. The inspector discussed staff records and the upcoming renewal application with the provider. The inspector arrived for the inspection at 8:45 am and departed at 9:55 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #: 8VAC20-800-100-A Description: REPEAT VIOLATION
Based on record review, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child?s attendance or within 30 days after the first day of attendance.
Evidence:
1. The record for Child 1 contained a documented first day of attendance as 02/19/2024; there was not a physical examination in the record for Child 1.
2. The record for Child 3 contained a documented first day of attendance as 05/01/2023; there was not a physical examination in the record for Child 3.Plan of Correction: Will get the information from the parents.
Standard #: 8VAC20-800-60-B Description: 3rd REPEAT VIOLATION
Based on record review, the provider failed to ensure that each child?s record shall contain all of the elements as required by the standards.
Evidence:
1. The record for Child 2 did not contain the following information: the name, address and telephone number of each custodial parent?s place of employment (an employer was listed but a telephone number was not provided); and the first date of attendance.
2. The record for Child 3 did not contain the following information: the name, address and telephone number of each custodial parent?s place of employment (an employer was listed but a telephone number was not provided).
3. The record for Child 4 did not contain the following information: the first date of attendance; and documentation of the review of the child?s emergency contact information as required.Plan of Correction: Will get the information from the parents.
Standard #: 8VAC20-800-320-B Description: Based on observations, the provider failed to ensure that child-resistant protective covers shall be installed on all unused electrical outlets accessible to children under five years of age.
Evidence: There was an uncovered electrical outlet in the foyer area of the home where the children enter and exit the home that was accessible to the children. There was an uncovered electrical outlet in the child care area of the home that was accessible to the children. All of the children in care were under five years of age.Plan of Correction: Will be covered today.
Standard #: 8VAC20-800-340-C Description: REPEAT VIOLATION
Based on observations, the provider failed to ensure that the bathroom shall contain toilet tissue, liquid soap and paper towels.
Evidence: The bathroom did not contain liquid soap.Plan of Correction: Will get today.
