Technical Assistance:
Reminder: It is incumbent upon the licensee to establish a procedure to ensure that staff records are always available for review by licensing representatives.

Comments:
Licensing Inspector arrived at facility at 8:30 and departed at 9:45 AM. At the time of the inspection, there were 53 preschool age children in care with eight staff. Children were observed eating breakfast and having circle time. Licensing Inspector arrived at main office at 12:20 and departed at 2:15 PM to review staff records for renewal.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of four children's records, it was determined that the facility did not ensure that all required information was present in each child's record. Evidence: Child #3, who was in care at the time of the inspection, did not have work telephone number and place of employment for the father. The father was listed as an emergency contact person with the same address as the child.

Plan of Correction: Facility responds: On July 17, 2015, child #3's mother added the father to the enrollment application and completed a new emergency contact form. The father's address was changed on both the enrollment application and the emergency contact form because the parents live in two separate locations. On July 17, 2015, child #4's mother removed the father from the emergency contact form because he is incarcerated for three years and does not live in the same household. The enrollment package will be accompanied by a letter giving specifics for completing the enrollment application and emergency contact form. Status - On-going monitoring

Standard #: 22VAC40-185-70-A
Description: Based on review of 16 staff records, it was determined that the facility did not ensure that all required information was present in each staff record. Evidence: Staff #11 and #14 did not have documentation of references as to character and reputation being completed prior to employment in their records at the time of the inspection.

Plan of Correction: Facility responds: Documentation of references regarding the character and reputation of staff are provided to Human Resources prior to employment of all staff and kept in their personnel record file. Staff records are kept at the central office and all required information is readily available to licensing inspectors. Status - On-going monitoring

Standard #: 22VAC40-185-510-A
Description: Based on a review of medication at the facility, it was determined that the facility did not ensure that medications had written parental authorization. Evidence: The medication authorization form for child #5 did not have the parental signature.

Plan of Correction: Facility responds: On July 17, 2015, the Family Services Worker called the parents of child #5 and set up a home visit the same day. The medication form (MAT) was signed by the parent and placed in the binder when they returned from their visit. MHK Head Start revised medication administration and storage of medication procedures to require the Center Director and Family Services Workers to review any MAT forms submitted by a parent before medication can be left in the care of the agency. Status - On-going monitoring
