An unannounced renewal inspection was conducted today, July 23, 2015 to determine the centers compliance with licensing standards. The inspector arrived at approximately 10:30 am and concluded the inspection at 1:15pm. The owner/director was present during the inspection. Upon the inspectors arrival, children were engaged in a variety of activities. The preschool and school age children were combined as a small number of children from each group were out of the building at swim lessons. Children were playing in centers consisting of blocks, constructing a race track, action figures, house keeping and coloring. Later the group of children returned from swim lessons and the school age group returned to their classroom. The two year old classroom was having circle time. Children sang songs and listened to a story. After the story children pointed out the parts of a book to the inspector, i.e.. spine, title page, author. Infants were observed playing on the carpeted floor with staff and toys. Staff were singing songs along with a children's music cd that was playing . Staff interacted with the children in a positive manner. The theme in the toddler classroom was animals. Activities included painting a hippo and making handprint animals. The toddlers were also playing with interlocking waffle blocks and with babies. Later children were observed on the playground climbing on the play equipment and with balls. Lunch time was observed. Bathroom breaks and hand washing were conducted before meals were served. After lunch children began transitioning to nap time. Soft music was played during this time. Lunch consisted of Mac and cheese, broccoli, mixed fruit and milk. The classrooms are decorated with artwork and have a variety of age and stage appropriate materials. Positive interaction was observed between staff and children. The playground was inspected during today's inspection. The center's first aide kit and emergency supplies were checked (see violation notice). The facility had posted the monthly menu, the License, the last inspection, evacuation maps and information, emergency numbers and various parent information. Field trip and summer permission slips were reviewed for the summer program. Five employee files (plus two follow up staff records) and seven children's records were reviewed during today's inspection. Multiple staff have current CPR, First Aid, and Daily Health Observation training. Emergency medication is not on site. There is no staff with current MAT training at the facility. Health inspection: 07/21/2015 Fire inspection: 07/25/2014 The last documented emergency evacuation drill was held on 06/25/2015. The two requires annual shelter in place drills for 2015 were completed on 06/10/2015 and 05/17/2015. Today the following ratios were observed: Caterpillars 1: (2 years) 8 children with 1 staff. School age: 10 children with 1 staff. Pre-school: (3-5 years) 15 children with 1 staff. (see violation notice) (Later 17 pre-school children with 2 staff, 6 school age children with 1 staff) Infants: (0-16 months) 6 infants with 2 staff. Toddlers: (16 months-2 years) 5 children with 1 staff. The violations from the previous inspection on 03/04/2015 were checked for corrections. There were three repeat violation found during today's inspection. A renewal application has been received by the Department of Social Service. Standard #: 22VAC40-185-140-A Description: Based on review of children's records, The facility did not obtain written documentation of a physical examination by or under the direction of a physician for child enrolled. Evidence: Child #1, with an enrollment date of 04/26/2015, did not have written documentation of a physical on record. Child #6, with an enrollment date of 02/02/2015, did not have written documentation of a physical on record. Plan of Correction: Per the Director: I will contact the parents today and have them provide a copy of the most recent physical examination for the child's record.
Standard #: 22VAC40-185-270-A Description: Based on observations, the facility did not ensure all areas of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: 1. One hole in the wall of the school age room (also where preschool children nap) was observed. The hole was approximately two inches. The hole created loose drywall and paint pieces that could be ingested by a child in care. 2. Two holes were observed in the wooden door frame of the door leading from the school age room to the playground area. One hole was approximately four inches and created a jagged edge of wood that was sharp and could snag the skin of a child in care. The second hole was approximately an inch. 3. The door leading outside from the preschool classroom has a metal push bar lock release. The metal bar has detached at one end exposing two metal pieces that extend outward and pose a risk of puncturing the skin of a child in care. The loose metal bar also created pinch points where it hangs down out of alignment. Plan of Correction: Per the Director: I will have my husband patch the holes. We will replace of have repairs made to the push bar on the door.
Standard #: 22VAC40-185-350-E-4 Description: Based on observation, the facility did not ensure the required ratio of children to staff for children aged three years to the age of eligibility to attend public school, five years by September 30, was met. Evidence: The preschool classroom had a total of fifteen children, aged three years through six years, with one staff when the standard requires one staff for every ten children. Plan of Correction: The Director went into the classroom and assisted until a second staff arrived. Per the Director, two staff were scheduled in the classroom however one staff left to attend a funeral. The staff left earlier then agreed leaving the classroom under staff for approximately ten minutes.
Standard #: 22VAC40-185-500-B Description: Based on observations, the facility did not ensure a nonabsorbent surface for diapering or changing was used. Evidence: The diaper changing pad in the infant room had a tear of approximately one inch along the top of the surface. The tear exposed the inner foam compromising the non-absorbency. Plan of Correction: Per the Director: I will purchase a new diaper changing mat for the infant room.
Standard #: 22VAC40-185-540-C Description: Based on observations, the facility did not ensure the required first aid kits included the minimum supplies. Evidence: The first aid kit used on the field trip did not contain the required adhesive tape, a thermometer and two triangular bandages. Plan of Correction: Per the Director: I will purchase those items at the store in order to have enough items for field trips and in the building.
