Comments:
An unannounced monitoring inspection was conducted on July 27, 2015 from approximately 4:20 pm to 6:20 pm. At the onset of inspection, six staff were present with forty-one children. Children were observed during snack and movie time. Four children?s records were reviewed on this date. Licensing Inspector reviewed supervision, activities, equipment, hand washing procedures, nutrition, injury report documentation, medication procedures, children and staff records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, liability insurance coverage, emergency supplies and required postings. Violations were reviewed with and verified by the Recreation Assistant Supervisor on this date. Please complete the columns for ?description of action to be taken? and ?date to be corrected? for each violation cited on the violation notice, and then return a signed copy to the licensing office within 10 calendar days of receipt. Description: Based on review, facility did not ensure 1 of 4 children's records has an immunization record prior to child beginning attendance. Evidence: 1. On July 27, 2015, the record for child #1 (start date June 2015) did not have an immunization record. 2. Missing documentation was verified by Recreation Assistant Supervisor.

Description: Based on review and interview, facility did not ensure there is at least one staff member on duty who has completed training in daily health observation within the past three years. Evidence: On July 27, 2015 from 5:15 pm - 6:00 pm, there were 2 staff present with children in care. There was not documentation that either staff has received training in daily health observation.
