22VAC40-191 Background Checks (22VAC40-191)

Technical Assistance:
1. Trash on the playground should be picked up daily before children are permitted to play. 2. Rugs and carpets that are stained should be replaced if not able to clean. 3. The wood fencing along the playground should be checked on a regular basis to ensure splinters and nails are not protruding, and for stability in general, as it is an aging fence.

Conducted an unannounced monitoring inspection. Observed 42 children + 6 direct-care staff. Additional staff were present and available to assist in classrooms as needed. Ratios and supervision were in compliance. Children were observed having outdoor play time, visiting the ice cream truck, washing hands, eating lunch, and taking naps. Classrooms were clean, safeguarded appropriately, and sufficiently supplied with toys and equipment. Lunch included: taco meat, cheese, tortillas, apples, and milk. Staff are currently trained to administer medications, to include Auvi-Q + rectal medication. Health and fire inspections were current. Required documents were posted. Description: Based on children's records reviewed, it was determined one child did not have current immunizations on-file. Evidence: The immunizations on-file for Child #1 were dated May 2014, when the child was 2 months old. Immunizations have not been updated every 6 months.

Description: Based on children's records reviewed, it was determined that one child did not have a physical exam on-file. Evidence: Child #1 enrolled in May 2014, and the 30-day grace period has expired.

Standard #: 22VAC40-185-270-A
Description: Based on observations made, it was determined that areas of the center were not maintained in a safe condition. Evidence: 1. A section of concrete has broken on the floor in the main hallway at the rear exterior door, and has left a hole approximately 9" long x 3-4" wide. A pipe has been exposed in the hole. This is in the path of travel to the rear playground. 2. A large chunk of concrete at the base of a pavilion pole was exposed and no longer covered by dirt on the playground. The concrete was approximately 7-8" in circumference. 3. Rusting nail heads were exposed on the wooden playground fence. The nails were popping from the wood and no longer smooth with the wood surface. 4. The wood playground fence was broken in two places along the top rail. The broken areas left exposed rusted nails, and cracked and splintered wood.
