Standard #: 22VAC40-185-60-A Description: Based on record review it was determined, the center did not ensure that each child's record contained all of the required components. Evidence: The record for child #4(enrolled 9/9/9) did not have documentation of a current annual update. The last update was dated 3/31/14. Plan of Correction: Child #4 file has been updated and the appropriate information received. Prevention Method: Before inputting a Club member into the membership tracking system the front desk staff and/or Unit Director will make sure all Club members will have updated membership form for the current year. Standard #: 22VAC40-185-270-A Description: Based on observation it was determined that the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: 1. There were areas of peeling paint through out the building to include the resource room, gym walls and doors, computer room, and both restrooms utilized by the children in care. 2. There were sections of broken plaster in the entry ways to both of the restrooms utilized by the children in care. 3. In the gym there was an unplugged large industrial sized fan next to an open outlet that had spaces wide enough for children's fingers to pass through. Children were observed in the above noted areas through out this inspection. Plan of Correction: Industrial fan has been moved into the electrical room where it will permanently located. Date of completion ? July 21, 2015 Painting and plastering will begin in the beginning of the school year. Completion date 10/29/2015 Prevention Method: After painting and plastering is done, the building maintenance will ensure any peeling of paint or plaster will be repaired immediately. Standard #: 22VAC40-185-280-B Description: Based on observation it was determined, the center did not ensure that all hazardous substances were kept in a locked place using safe locking method that prevents access by children. Evidence: The door to the hallway off of the gym was unlocked which allowed access to a storage area which was also unlocked as the door was propped open by a large container of paint, in the storage area there was containers of: -floor stripper -floor polish -glass cleaner -pH balancer -Pink Lotion Soap -several spray bottles with cleaning agents All of the above noted chemicals were unlocked and accessible to children in care and carry the MSDS label of "Keep Out Of The Reach Of Children". Plan of Correction: Building maintenance will monitor the area/doors to ensuring all doors leading into the locker room doors are locked. Prevention Method: Check doors at the beginning of the day and at the end of the night to ensure all doors leading to locker rooms are locked. Prevention Method: Move all containers of floor stripper, floor polish, glass cleaner, and soap to a separate closet, which we (Unit Director and Building Maintenance) will monitor to make sure the closet is locked at all times.
