The center is now due for an updated fire inspection. Revisions to central registry check form (and fee increase) discussed with program director. Written information left.
An unannounced monitoring inspection was conducted on 8/3/2015 from 9:00am - 12:50pm. The following staff to child ratios were observed: *Infants - 7 x 2 staff *Two's - 12 x 2 staff *Three's - 10 x 2 staff *Four's - 10 x 2 staff *SA - 20 x 2 staff Children were observed during morning program time, lunch and nap. Diapering was observed in the toddler class. Records were reviewed for five children and three staff. Additionally reviewed: medication, emergency supplies, transportation & field trip procedures, injury documentation, documentation of emergency practice drills, hand washing and posted menu.
Standard #: 22VAC40-185-130-B Description: Based on record review and interview, the center failed to obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: 1. The most recent immunizations for child #1 were administered on 8/4/2014 according to documentation in the child's record. 2. Administrative staff report updated immunizations have not been obtained for this child who is under the age of two years. Plan of Correction: The parent will be contacted today and asked to provide a copy of updated immunizations by the end of this week.
Standard #: 22VAC40-185-280-B Description: Based on observation, the center failed to ensure that hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. If a key is used, the key shall not be accessible to the children. Evidence: 1. The closet in the three-year old class was unlocked at 9:10am when this classroom was viewed. Five bottles of cleaning agents were stored on the shelf in this closet. 2. The kitchen was open and unattended when viewed at 12:00pm. The cabinet located under the hand washing basin was unlocked. 12 bottles of cleaning agents were stored inside of this cabinet. Plan of Correction: The closet in the three-year old class was locked up immediately. Additionally, the kitchen door was closed and locked during the inspection. All staff will be reminded that all hazardous chemicals must be maintained in a locked location.
