Comments:
An unannounced complaint inspection was done today July 23, 2015 beginning at 10:20 AM and ending at 4:30 PM. The allegation was received in the licensing office on June 29, 2015. The allegation indicated ratios were not met in the gym as there was one staff with twenty-five children. Also, the ratio was not met on the playground on June 25, 2015 as there were only two staff at pickup time. No age group was given. Also, of concern was that only the snack menu was posted and there was not enough milk and food served, the snack menu consisted only of saltines and juice which was very acidic and the younger children were rushed through lunch so the school age children could leave for field trips so there was not enough time for the little ones to eat. The last component of the complaint was that the Director was never there. The inspector interviewed staff during the inspection. Based on information obtained from staff and observation by the LI, it was determined that the complaint was valid in the area of ratio only. No other violations were cited. Description: Based on observation the center failed to maintain the correct ratio of 1 staff to 18 children in a mixed age group. Evidence: 1. During a complaint inspection the LI upon arrival at 10:20 AM observed 30 children in the gym with staff # 1. The ages of the children ranged from age 5-11 per discussion with the staff member.

Plan of Correction: Will take measures to have a "floater" or on call person to have available immediately. Day of complaint inspection, we had no-show staff member. All ratios will be made and enforced during all business hours.
