Comments:
An unannounced monitoring inspection was conducted on 1/22/20 from 10:00 am until 11:00 am at Gemisha Williams family day home to review the subsidy health and safety requirements. At the time of the inspection no children were in care. The provider, adult member of the household, a child that lives in the home and an adult visitor were present. Three children are enrolled. The sample size consisted of the three children's records, a review of the provider's record and the adult member of the household's record. The home was inspected along with the first aid kit and evacuation drills. A violation was found during this inspection and is documented on the violation notice. Description: Based on record review and interview, the vendor failed to obtain documentation of a child's current immunization record before the child's first date of attendance.

Evidence:

1. The records for the three children enrolled were reviewed. The record for child 1 did not contain documentation of immunizations.
2. The vendor stated she thought she had the documentation but she could not find them.

Plan of Correction: The parent of child 1 will be advised to provide a copy of the immunization record. In the future the vendor will ensure the immunization records are obtained for children prior to the first date of attendance.
