22VAC40-185 SPECIAL CARE PROVISIONS AND EMERGENCIES.
An investigation was conducted based on a self reported incident related to medication administration. .
After interviews with staff and a review of the medication administration documentation, it was determined that there had been a medication error. See Violation Notice.
Standard #: 22VAC40-185-340-A Description: Based on interviews with staff and a review of the medication administration documentation, it was determined that the Center failed to provide care and protection to a school age child. Evidence: Child A, age 7 years, was scheduled to be given medication (Adderall) every Monday, Tuesday and Wednesday but only if he was attending a tutoring class on those days. Staff #1, who is Medication Administration (MAT) trained, gave the child medication on Thursday, August 6th and Friday, August 7th in error. Staff #1 misread the medication label and did not check the label against the permission form. Plan of Correction: All MAT Trained staff will be retrained to follow guidelines and double check the instructions.
Standard #: 22VAC40-185-510-L Description: Based on a review of documentation of medication administration it was determined that the medication administration record was not complete.
Evidence: Staff #1 documented that medication was given to child A on Thursday August 6th and Friday August 7th but did not document that the medication had been given in error. Staff #2, who was not present at the Center on August 6th and 7th, realized the error on August 11th but did not document the error.
Plan of Correction: The medication error was documented by the Assistant Director on August 11, 2015. Staff will be retrained in medication administration documentation.
