An unannounced monitoring inspection was conducted on 08/13/2015 from 9:05am - 11:10am. During the inspection there were 109 children ages 2 months to 12 years old in care with 25 staff. A tour of the facility was conducted and children were observed in a variety of activities in the classrooms including a singing, dancing, story time, reading, arts and crafts, playing with blocks and puzzles and sensory activities, and playing outside. Records were reviewed for seven children and five staff. Medication was reviewed along with emergency procedures and emergency supplies. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).
Standard #: 22VAC40-185-140-A Description: Based on a review of seven children's records, it was determined that the facility did not ensure that each child in attendance had a completed physical within one month of attendance. Evidence: 1. The record for child#3 (date of enrollment 8/20/14) did not contain documentation of a physical examination. 2. Staff #6 reviewed the record for child #3, and confirmed that the physical examinations had not been received for child #3. Plan of Correction: The missing physical form was an oversight on the administration's part. The mother was contacted at the time of the inspection and the child has an appointment next week to complete a physical form. The administration will change the way files are audited to ensure that oversights like this are not missed.
