22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was conducted today by the licensing inspector. Some but not all standards were checked today. A sample of staff and children's records were reviewed today and observations were done in all classrooms. There was no medication on site for children in care today. All required information was posted. The playground was also inspected today. menu today; breakfast fruit ring cereal, pineapple tidbits, milk afternoon snack wheat crackers, milk lunch chicken alfredo, pasta, green beans, applesauce, milk evacuation drill 7/31/15 all months done emergency preparedness/shelter in place drill - Two need to be completed in 2015 health inspection - 3/24/15 fire inspection - 10/15/14 liability insurance expiration 12/26/2015 staff/child ratios observed: schoolagers 2:22 Children had just returned from a field trip to see a movie and were preparing for lunch 3/4's 3:16 This group has just come in from the playground and were transitioning to lunch. 2's 2:16 The group had finished lunch and were beginning nap time toddlers 2:7 Most of the children were napping. Footprint puppies done by the children were posted. infants 1:3 Two of the children were asleep and the staff person was attending to the infant who was awake. There is a feeding table in the infant that is not being used at this time due to the ages of the infants in care. Prior to use by the children, the safety straps need to be placed in the seats. Please be reminded that children may not use the slides/playground equipment until the amount of resilient material in place has been corrected. approximate time on site: 11:45 am - 3 pm

Standard #: 22VAC40-185-160-C
Description: Based on review of a sample of staff records, the facility did not obtain TB test results for a staff person. Evidence: Staff 2. hire date 6/8/15 A document indicating a TB test had been done on 10/6/14 was on file. However, there was no documentation indicating the results of the TB test.

Plan of Correction: We got staff person to call the place of previous employment and they are faxing it tomorrow.

Standard #: 22VAC40-185-270-A
Description: Based on inspection of the playground, the facility did not maintain areas in a safe condition. Evidence: Areas of the wooden landscape timbers on the perimeter of the right side of the playground have splintered with sharp areas accessible to children. There is a rectangular sandbox on both the left and right side playgrounds. There were wasps in both sandboxes burrowing into the sand and flying around the sandboxes creating a risk to children.

Plan of Correction: The plan of action is to contact the owners to see if that is part of their landscaping reasponsibility and have them treat it. If not we will have our own maintenance person to correct and treat the problems by Sept 8th. We will cover/close off the sandboxes on the playground today.

Standard #: 22VAC40-185-330-B
Description: Based on inspection of the playground, the facility did not ensure there is enough resilient surfacing in the fall zones that comply with minimum safety standards. Evidence: The fall zones around the slides on both areas of the playground have 1-2 inches of resilient material. The landscape fabric is visible in areas of the fall zones.

Plan of Correction: We will have our own maintenance person to correct the problems with the mulch.

Standard #: 22VAC40-185-340-F
Description: Based on observation, the facility did not ensure sight and sound supervision in the infant room. Evidence: One staff was in the large infant room area with one child. Two infants were asleep in the enclosed area on the other side of the sink. The enclosure consists of a clear glass upper wall and the door to the room was open. Standards require that a staff person be present in each classroom where infants/children are in care (under 10 yrs of age). The two children sleeping in their cribs could not be seen by the staff in the large room.

Plan of Correction: The staff person and the third child moved into the enclosed room where the other two children were napping. This corrected on site we also agreed to rearrange the room to assure there is always sight sound supervision.
