Comments:
Arrived 8:50 am Departed 11:35am An unannounced renewal licensing study was conducted today. Three infants were present with two staff. Eight toddlers and twos were present in Early Head Start with two staff. Two two-year-olds were present in the day care twos' class with one staff member. Four preschoolers were present with one staff member. The children were reading books, playing in centers and outdoors and eating lunch. Three children's files were reviewed for compliance. One staff file was reviewed. Thank you for your cooperation. Please contact me if I may be of service.

Standard #: 22VAC40-185-270-A
Description: Based on a tour of the playground, the facility did not maintain equipment in a safe and operable condition. Evidence: 1. On the large climber on the top deck , the yellow panel connected to the green pole has a loose bolt.

Plan of Correction: The Early Head Start Coordinator will put in a maintenance request the repair of the bolt.

Standard #: 22VAC40-185-330-B
Description: Based on a tour of the playground, the facility did not maintain the required resilient surface material. Evidence: 1. There is less than one inch to two inches of wood mulch under climbing equipment.

Plan of Correction: The Facilities/ Maintenance Coordinator has ordered new resilient surface material for the playground. It will be delivered on Tuesday, August 18.

Standard #: 22VAC40-185-510-N
Description: Based on a tour of the classrooms, the facility did not return medication to the parent at the end of the prescribed administration period. Evidence: 1. The LI observed two bottles of medication (Benedryl and Children's Fever Reducer) in a locked cabinet for child #1 that are dated 10/14/14.

Plan of Correction: The lead teacher will return the medication to the parent today.
