22VAC40-191 Background Checks (22VAC40-191)

Arrived 10:25am Departed An unannounced monitoring inspection was completed today. Six infants were present with two staff. Eight toddlers were present with two staff. Eight twos were present with one staff member. Six children were in the the threes' class with one staff member. Seven four-year-olds were present with one staff member. Six schoolers were present with one staff member. The children were eating lunch, playing outdoors and resting. Three staff files were reviewed and five children's files were reviewed. Standard #: 22VAC40-185-270-A
Description: Based on a tour of the outdoor playground the facility did not maintain equipment in a safe condition. Evidence: 1. The LI observed that on the upper and middle slide decks the green plastic hoods for the slides each have a missing bolt.

Plan of Correction: The bolt was replaced and checked periodically. It will be replaced again, along with the other bolt.

Description: Based on observation, the facility did not keep all children in sight and sound supervision. Evidence: 1. As staff #4 and the pre-3 class went down the hall to the bathroom, child #6 stayed in the room, without staff supervision, for less than 30 seconds, and then joined the group.

Plan of Correction: The director will monitor the room and will review the sight and sound supervision policy with the staff member.

Standard #: 22VAC40-185-560-N
Description: Based on observation, the facility did not require all children to sit down to eat. Evidence: 1. Child #6 was observed running down the hall to the bathroom while eating a piece of bread.

Plan of Correction: The program director will insure that the children eat only while seated at the table.
