22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 10:00am-11:45am. Upon arrival there were 72 children in care with 13 staff members present. A sample of 2 staff and 4 children's records were reviewed in addition to 4 medications. Children were observed playing outside, doing an art activity and babies were observed being changed and playing on the floor.
Standard #: 22VAC40-185-60-A Description: Based on a record review the center failed to ensure each child enrolled had all required information. Evidence: Child #1 did not have documentation of one of the 2 required emergency contact information. Plan of Correction: This was an oversight. The director/owner will obtain this information today and document it in the record.
Standard #: 22VAC40-185-270-A Description: Based on observation the center failed to ensure areas and equipment of the center were maintained in a safe and operable condition. Evidence: 1. One of the cribs assigned to a child inc are had chipped and peeling paint on the inside railing of the crib. Paint chips fell off when the inspector touched the railing. 2. There was peeling paint in reach of the children inc are, along the walls in the toddler room. Plan of Correction: The owner/director will look into getting crib railing protectors to prevent chipping and also cover the current chipped area. All areas in the center with peeling paint will be repainted.
Standard #: 22VAC40-185-330-B Description: Based on observation the center failed to ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99. Evidence: On the playground there was 5 " of resilient surfacing in the fall zones of the swing set and visible grass patches. there was 3 inches of resilient surfacing underneath the pull-up bars with a 4' fall zone. A 6' fall zone with 6" of resilient surfacing is required for these pieces of equipment. Plan of Correction: the owner is aware the playground is in need of resilient surfacing. the person who maintains her playground has been out of town. Resilient surfacing will be ordered and placed in the required areas as soon as the maintenance person returns from being out of town.
