Comments:
The licensing inspector conducted an unannounced monitoring visit from approximately 2:15 until 3:45. Children were observed napping, having snack, lining up for a bathroom break and washing hands. Four staff were present supervising eleven children at all times. Three children's records and two staff record were reviewed. Violations are identified on the violation summary. Standard #: 22VAC40-185-270-A
Description: Based on observation, there was black landscaping cloth on the playground causing a tripping hazard. Evidence: 1. The inspector observed landscaping material that was loose on the playground next to the train and also next to the building to the left of the steps that causes a tripping hazard.

Plan of Correction: The director will cover the material with mulch and monitor the playground to make sure it does not create a tripping hazard.

Standard #: 22VAC40-185-330-B
Description: Based on measurement, the center failed to have the required amount of resilient surfacing on the playground. Evidence: 1. Resilient surfacing (mulch) measured approximately 2-3 inches surrounding the race car bouncer and surrounding the four seat bouncer. Resilient surfacing needs to be at least 6 inches.

Plan of Correction: Children will not be allowed to play on the equipment until the mulch is applied at the required depth.

Standard #: 22VAC40-191-60-B
Description: Based on record review, the facility did not have a completed sworn disclosure for an employee. Evidence: 1. The inspector reviewed two staff files. One of two files did not have a completed sworn disclosure. Staff #1 had signed the sworn disclosure, but not answered the questions.

Plan of Correction: Corrected during inspection.
