Consultation was provided on Standard 550.M - Documentation of children's injuries.
An unannounced complaint inspection was conducted on 8/4/2015 in response to a complaint that was received in the licensing office on 7/23/2015. The complaint alleged a lack of adherence to regulations regarding supervision, diapering and injury documentation. Based on the information gathered, the complaint regarding diapering and supervision is not valid. The allegation regarding insufficient written documentation of children's injuries is found to be valid. Please complete the "plan of correction" for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word "corrected" is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).
Standard #: 22VAC40-185-550-M Complaint related: Yes Description: Based on record review and interview, the center failed to maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include the following: *Date and time when parents were notified; *Staff and parent signatures or two staff signatures; and *Documentation on how parent was notified. Evidence: 1. Center staff could not locate written documentation of an injury received by a child, while in attendance, on the morning of 7/31/2015. 2. Center staff could not locate written documentation of any injury received by a child, while in attendance, on the morning of 7/23/2015. 3. A sample of 17 injury reports reviewed for the months of July and August 2015 found the following documentation lacking: a) Two injury reports lacked a parent signature or second staff signature; b) Seven injury reports lacked documentation of the time when the parent was notified of the injury; c) Five injury reports lacked documentation of how the parent was notified of the injury. Plan of Correction: Staff will be retrained on all aspects of incident reporting. All written incidents will be reviewed on a daily basis with follow-up of weekly filing. Training will be completed by September 4, 2015.
