Technical Assistance:
Staff files were reviewed at the administrative site earlier in July.

Comments:
An unannounced monitoring study was conducted from 9:15-10:30. Children were observed transitioning into program, staff and child interactions, transition to the bathroom, free play and preparation for outside time. Five staff and two volunteers were present with twenty children. The staff had prepared for the morning preschool program with set-up art and crafts, music playing and materials set out to play with. The outside play time was adjusted to avoid the hotter part of the day. A special visitor was scheduled to visit later in the morning. Emergency preparation and medication were reviewed. Areas of non-compliance are identified in this report. Please sign and date the report and return it to me within 10 calendar days from date of receipt. Standard #: 22VAC40-185-60-A
Description: Based on record review, 1 of 3 records was incomplete. Evidence: Child #1 did not have the 2 emergency contacts required on the child information form. The parent added the father and mother's name rather then 2 designated people to call if the parent cannot be reached. Based on child information form, the center could not take the action listed by the parent for a documented allergy. Evidence: Child #2 listed an Avi-Q pen be given on the child's physical but no emergency medication was present for the staff to use in case of an allergy exposure.

Plan of Correction: 1. Parent asked for additional contacts. 2. Parent was asked to provide the medication or revise form noting medication not needed.

Description: Based on observation, the most recent inspection notice was not posted. Evidence: The inspection notice was dated 2014 rather than the inspection conducted on 2/2015.
