Comments:
An unannounced monitoring inspection was conducted from 10:00 am - 12:45 pm. There were 125 children in care being supervised by 16 staff. The children were observed in outdoor play, story time, and art projects. Lunch served today is tomato soup with ground hamburger, mixed berries, bread sticks, and milk. Fifteen children's records were reviewed and in compliance. Nine staff records were reviewed and in compliance. Three medications were reviewed and in compliance. Standard #: 22VAC40-185-280-B
Description: Based on review of the center it was determined that not all hazardous substances were kept in a locked area that prevents access by children. Evidence: 1. The utility closet on the infant/toddler wing was unlocked and accessible to children. Inside the closet was 2 bottles of disinfectant. 2. In the Pre-K classroom there was a bottle of hand sanitizer on the teacher's desk.

Plan of Correction: 1. Administration will contact maintenance to repair the lock. The disinfectant was moved to a locked cabinet during the inspection. 2. The hand sanitizer was placed in a locked cabinet during the inspection.

Standard #: 22VAC40-185-500-B
Description: Based on interview and review of the diaper area in the toddler room it was determined that not all procedures for diapering were followed. Evidence: 1. Staff A was not using the foot operated trash can for diapering. 2. The LI (Licensing Inspector) observed items on the changing table not used for diapering such as: attendance clipboard, binder, toys, and tissues.

Plan of Correction: Staff will be retrained to use the foot operated trash can for changing as well as only allowing items used for diapering on the changing table.
