Comments:
An unannounced monitoring inspection and complaint inspection was conducted today by the licensing inspector. Some but not all standards were checked today. The vehicle will not be used for transporting children until school begins next week. All required items were posted. The center director was present and assisted during the inspection. Previous violations have been corrected. Some but not all standards were checked today. The children's art work was displayed throughout the center and observations were done in all classrooms. The floor in the young toddler room will be replaced this weekend.

staff/child ratios observed:
pre K 1:9 Children were having snack then began reading time.
schoolagers 2:19 The children had finished snack and were relaxing on the floor reading books of choice.
3's 2:15 Children were grouped together with 2 staff and were reviewing colors, shapes in Spanish and their ABC's. They also talked about family member names in Spanish.
2's 3:24 This group was finishing up snack, diapering and washing hands.
young toddlers 2:8 The children were playing with balls and then legos. Staff were on the floor playing along with the children.
older toddlers 2:10 The children were playing in two groups. One group was painting blue and green pictures while the other group was using flash picture cards to identify various animals and and a variety of other objects.
infants 2:10 Staff were holding, rocking, feeding diapering and playing with the babies. The children were on the floor, in a crib, in an infant seat or boppee and interacting with staff.

Description: Observation: Based on review of a sample of children's records, the facility did not have verification of age/identity on file for a child. Evidence: child #2 start date 6/15/15 did not have verification of age and identity on file.

Plan of Correction: The parent was contacted today. If the information is not brought in tomorrow, the police will be contacted as required by the Code of Va.

Standard #: 22VAC40-185-540-E
Description: Based on observation, the facility did not have required nonmedical emergency supplies on site. Evidence: There was no flashlight on either floor of the building.

Plan of Correction: I will buy flashlights tonight and place on on each level of the building.
