Comments:
An unannounced monitoring inspection was conducted from 8:30am-10:30am. Upon arrival there were 88 children in care with 10 staff present. A sample of 2 staff records were reviewed and transportation was discussed. The center will not be using vans for the rest of the summer. Children were observed playing in centers and on the playground.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation the center failed to ensure areas and equipment of the center were maintained in a safe and operable condition. Evidence: In the school age room there was a large backed television on a cart with wheels. It was not secured to the cart and posed a tipping hazard.

Plan of Correction: The owner/director will remove the television from the room and will also talk to staff about identifying tipping hazards.

Standard #: 22VAC40-185-280-B
Description: Based on observation the center failed to ensure hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: In the classroom next to the front desk there was a bottle of correction fluid on the teacher's desk, in reach of the children in care.

Plan of Correction: The bottle of correction fluid will be removed from the classroom.

Standard #: 22VAC40-185-340-F
Description: Based on observation the center failed to ensure children under 10 years of age were always in sound supervision when using the restroom. Evidence: At 8:45am the inspector observed a 5 year-old child enter an empty classroom to use the restroom. The child stated he was sent from the front preschool room. The child finished in the restroom at 8:48am and walked back to class. Due to the proximity of the restroom to the classroom the child was not in sound supervision of a staff member.

Plan of Correction: The owner/director misunderstood the standard and thought all school-age children could attend the restroom without sight and/or sound supervision. The owner/director addressed the incident with the staff during the inspection and plans to have a staff meeting on Thursday.
