Comments:
A complaint investigation was conducted on 8/10/2015 from 9:45 a.m. to 12:05 p.m. to investigate a complaint regarding forbidden actions. Interviews were conducted, staff records, and documentation were reviewed. The complaint regarding forbidden actions was determined to be valid.

Description: Based on review of the Younger School Age Room (Kindergarten & 1st grade) classroom it was determined that not all children under 10 remained in sight and sound supervision. Evidence: The LI (Licensing Inspector) observed a child from the younger school age room (kindergarten & 1st grade) walking in the hallway alone returning to the classroom from the rest room without adult supervision.

Plan of Correction: Administration will retrain staff regarding sight and sound supervision.

Description: Based on staff interviews it was determined that the facility failed to ensure staff do not engage in forbidden actions towards children. Evidence: It was reported to the LI (Licensing Inspector) that on 7/29/2015 milk was withheld from child A due to the fact that the child did not eat their snack. Staff A offered snack to child A who did not eat the snack. Child A requested milk from staff A but was refused because the child did not eat their snack.

Plan of Correction: Staff will be retrained and will no longer withhold beverages based on if the meal has been consumed. The meal and beverage shall be offered to each child at the same time.
