The revised central registry form and fee increase was discussed.

Comments:
An unannounced renewal inspection was conducted on 9/3/2015 from 10:25am - 12:55pm. The following staff to staff ratios were reviewed: *2's - 12 x 2 staff *3's - 14 x 2 staff *4's - 19 x 2 staff *SA - 29 x 2 staff *SA - 18 x 1 staff Children were observed during morning program time, outdoor play, and nap. Records were reviewed for four children and five staff. The inspector additionally reviewed: transportation and field trip procedures, medication, emergency supplies, written emergency plans and practice drills, the posted menu, hand washing, and the updated injury prevention plan.
