Standard #: 22VAC40-185-140-A Description: Based on a review of children's records, the licensee did not obtain a physical examination for one child. Evidence - The record for child #7 did not contain a physical examination. Plan of Correction: Will get child's updated physical exam and include in their file.
Standard #: 22VAC40-185-60-A Description: Based on a review of children's records, the licensee did not obtain all of the required information. Evidence - The records for children #2, #6 and #7 did not contain the child's doctor's name and telephone number. Plan of Correction: Will get and update children's file with doctor's name & telephone number.
Standard #: 22VAC40-185-70-A Description: Based on a review of staff records, the licensee did not obtain all of the required Background checks for one staff member. Evidence - The record for staff member #3 did not contain the Virginia State Police Check or the Child Protective Services Central Registry Check. The staff member was hired on 6/1/2015 and is no longer eligible to work on site until the checks are received. Plan of Correction: Staff member is not present until 24 hour CPS Check comes in. Sent off for new VA Police Check 9/2/15.
Standard #: 22VAC40-185-270-A Description: Based on observation, the licensee has not maintained the center in a clean, safe condition. Evidence - The bathroom shared by the school age and pre - K classes is dirty and in poor repair with missing molding around the bathroom divider. The area outside of the bathroom on the pre - K side is dirty. The carpets in the two year old room are dirty and stained. The walls throughout the center are dirty. Plan of Correction: Will do cleaning of center, carpets and walls. Put in work orders with maintenance for carpets and repair Pre-K bathroom area.
Standard #: 22VAC40-185-280-B Description: Based on observation, the licensee did not keep all hazardous substances in a locked place. Evidence - The cabinet containing cleaning supplies in the school age classroom was locked with a plastic child proof lock intended to prevent very young children from accessing the items in the cabinet. School age children would be able to unlock the device. Plan of Correction: Will get and install appropriate lock & key lock for school-age cabinet.
Standard #: 22VAC40-185-430-B Description: Based on observation, the licensee did not provide age appropriate materials in the two year old room. Evidence - There were beads observed on the counter by the sink labeled as intended for use by children 6 years and older. There was a container of green glitter observed on the counter by the sink labeled as intended for use by children 3 years and older. There were pom poms and dried pasta observed in the sand table. Both items could pose a choking hazard and would fit in a choke tube for checking items for appropriateness. Plan of Correction: Remove all choke hazards from the Two's classroom. Use choke test tube to "test" further items. Teachers and management will read all material labels for age.
Standard #: 22VAC40-185-510-A Description: Based on a review of all of the medications on site, the licensee did not have current parental authorization for all of the medications. Evidence - The medication authorizations for children C, D, E and F were expired. Plan of Correction: Remove medicines and return to parents or have parents complete updated medicine forms.
Standard #: 22VAC40-185-510-E Description: Based on a review of medications, the licensee did not label one of the medications. Evidence - The non prescription medication for child H was not labeled with the child's name. Plan of Correction: Label all medications with children's names.
Standard #: 22VAC40-185-510-N Description: Based on a review of medications on site, the licensee did not return one medication to parents within 14 days of the child leaving the center. Evidence - The medication for child J was not returned to the parents. The child stopped attending a month ago. Plan of Correction: Contact parent and have them pick-up the medicine.
Standard #: 22VAC40-185-560-M Description: Based on observation, the teacher in the Pre - K classroom did not sit with the children during meals. Evidence - The teacher in the Pre - K classroom was observed putting out the children's cots and serving food but not sitting down with the children during lunch. Plan of Correction: Pre-K teacher was re-trained on policy and will sit with class at lunchtime.
