1. Reviewed written injury report writing, with specific emphasis on the intent of "action taken to prevent future incidents." Statements such as "talked to child about sharing" and "talked to both children about using their words" would not be considered a preventive action for 2 year old children; and may not be the most appropriate statement to include on the report of the child that was bitten.

Conducted an investigation into allegations that biting incidents were not handled appropriately. Interviews conducted and records reviewed. Staff reported appropriate prevention techniques being implemented, and written injury reports were maintained for two recent biting incidents. Although there was a documentation violation found, there was not a preponderance of evidence to support the allegation therefore this complaint is deemed to be not valid/unfounded.

Description: Based on records reviewed, it was determined that 1 of 2 written injury reports did not include documentation of the "time" the parent was notified. Evidence: Report written 8/6/15 for Child #1.

Plan of Correction: The staff member was retrained and informed on policy.
