Technical Assistance:
Today we discussed use of space on the first floor, direct access to outside and consult with the Fire Marshal's office before moving the location of the infant room. Please remember that any changes to the program should be highlighted in the renewal application as requested changes. This will include a change in ages of the children to be served.

An unannounced renewal inspection of the facility and records was conducted from 9:15am ? 11:30am. There were 3 children in care, ages 13 months, 19 months, and 2 years with 2 staff members. Interviews were conducted with children and staff present. Seven children?s records and two staff records were reviewed. Children were observed in transitions, hand washing, rest room routines, teacher and self-directed activities.. The center does not transport children in care. Staff report there is not any medication being administered at this time. The inspector reviewed the center first aid kit, emergency radio and flashlight, insurance documentation, fire and health inspection and emergency evacuation drills. Violations were found in the following areas and appear as violations on the violation notice: staff record and children?s records.

Standard #: 22VAC40-185-160-A
Description: Based based on record review and interview the center failed to ensure that each staff shall submit documentation of negative tuberculosis screening no later than 21 days after employment and shall have been completed within 12 month to or 21 days after employment. Evidence: 1 - The hire date for staff 2 is 5/5/15. 2 - The record for staff 2 contains TB test results dated 8/22/12. The results are older than 12 months prior the date of employment 3 - Staff 2 confirmed that the test results are dated 8/22/12.

Plan of Correction: Center management will obtain the information from the staff 2. Going forward center management will obtain the required information within the required time frames.

Standard #: 22VAC40-185-60-A
Description: Based on record review and interview the failed to ensure that children's records contained all required information. Evidence: 1 - The record for child 1 does not contain documentation of the first date of attendance. 2 - Staff 1 confirmed that the information was not present. Staff 1 went on to state that child 1 was no longer enrolled in the child care center. 3 - The record for child 1 does not contain documentation of last date of attendance. 4 - The record for child 2 does not include addresses with the emergency contacts listed. The child's documented date of enrollment is 522/15. 5 - The record for child 3 contains only 1 emergency contact. The child's documented date of enrollment is 9/8/15.

Plan of Correction: Center management will document the first and last dates of attendance for child 1. Going forward center management will create and implement a method for clearly documenting the first and last dates of attendance for each child.
