Technical Assistance:
8VAC20-800-750. Topical skin products. A. When topical skin products such as sunscreen, diaper ointment and lotion, oral teething medicine, and insect repellent are used, the following
requirements shall be met: 1. Written parent authorization noting any known adverse reactions shall be obtained at least annually; 2. The product shall be in the original container and, if provided by the parent, labeled with the child's name; 3. Manufacturer's instructions for application shall be followed; and 4. Parents shall be informed immediately of any adverse reaction. A copy of the OCCHS model form was included in TA for provider.

Comments:
An unannounced, on-site renewal inspection was initiated on August 15, 2024 and completed on August 15, 2024. The on-site inspection began at 10:00 AM and ended at 11:20 AM. The inspector reviewed compliance in the areas listed above. There was one child present and two caregivers; the point total was 4. The inspector reviewed three children?s records and two staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on August 23, 2024. A POC submitted after this date will not appear on the public website.

Description: Based on observation and interview, the provider did not ensure that topical skin products are used, as required.

Evidence:
Two children?s diaper creams were observed. The provider stated that a written parent authorization was not obtained prior to use.
