Thank you for your time and assistance during the unannounced monitoring inspection conducted on September 15, 2015, between the hours of approximately 9:53 a.m. and 1:58 p.m. Nineteen (19) children were enrolled and eighteen (18) children were in the care of four staff members during the on site inspection. Four children's records and five staff records were reviewed and/or updated. One child's medications and accompanying authorization forms were reviewed. Safety equipment (first aid kit, flashlights and radio ), posted information (license, licensing inspection documentation, emergency plan & diagram, emergency telephone numbers and classroom schedule) and available documents (emergency evacuation drill & shelter-in-place records, agency inspection reports, insurance verification and injury report forms) were observed/reviewed. The daily schedule accurately reflected the activities provided. Lunches were provided by the children's parents. Observed staff/child interactions were positive and supportive of children's growth and development. The facility was observed to be clean and supplied with a variety of age/stage appropriate learning and play materials/toys. Program observation was completed. Activities observed on this date included indoor play and "theme based" activities (play doh, toys and painting self-portrait pictures based on a story read by the teacher). The theme for the week was "All About Me." Also observed were: diapering, restroom and handwashing procedures; lunch; stories; and naptime. Previously cited violations were reviewed for compliance; no repeat violations were cited. Exceptions to program compliance were noted on the violation notice. Standard #: 22VAC40-185-330-B Description: Based on playground observation and discussion with staff on September 15, 2015, the program failed to ensure a fall/use zone of six feet surrounded both the free-standing slide and the castle/slide. Evidence: 1. Observation of the free-standing slide, on this date, revealed that the fall/use zone surrounding the slide was approximately three to four feet. 2. Observation of the fall/use zone surrounding the castle/slide revealed that there was only approximately four feet from the castle tower to the wooden border and approximately four feet from the castle slide to the tree limbs. 3. Staff #1 confirmed that the fall/use zones surrounding the free-standing slide and the castle/slide lacked six feet of clearance. Plan of Correction: The castle/slide will be re-positioned to ensure a fall/use zone of six feet surrounding the play structure. The free-standing slide will be relocated (mulched area possibly expanded) to ensure a fall/use zone of six feet.
Standard #: 22VAC40-185-510-C Description: Based on observation of medications and discussion with staff on September 15, 2015, one child's prescription medication had an expiration date of January 2015. Evidence: 1. Observation of child E's inhaler (Albuterol), on this date, revealed that the medication expiration date was January 2015. 2. Staff #1 verified the expiration date of the medication. Plan of Correction: The child's parent has been contacted and another inhaler will be obtained. In the future, all medications will be periodically checked to prevent the use of outdated medications.
Standard #: 22VAC40-185-510-G Description: Based on observation and discussion with staff on September 15, 2015, the program failed to obtain a prescription label for one child's medication. Evidence: 1. Observation of child E's inhaler, on this date, determined that a prescription label was not obtained. 2. Staff #1 confirmed that the prescription label for the inhaler was not obtained. Plan of Correction: The child's parent has been contacted and the prescription label will be obtained. In the future, the prescription label will be obtained whenever a prescription medication is brought to the center.
