Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. Comments:
An unannounced, on-site monitoring inspection was initiated and completed on August 14, 2024, as a part of licensure period. The on-site inspection began at 10:00am and ended at 11:30am. The inspector reviewed compliance in the areas listed above. There were 11 children present, 3 caregivers and 1 provider. The inspector reviewed 5 children?s records, 1 provider record, 3 caregivers records and 3 household members records on-site. This inspection included document review, tour of the home, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/28/2024. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-800-100-A
Description: Based on records review and interview, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child?s attendance or within 30 days after the first day of attendance.
Evidence:
Child #4 (start date 8/16/2021) did not have documentation of a physical examination in the file.

Plan of Correction: Reach out to the parent to get the physical exam today.

Description: Based on record review and interview, the provider did not have the required information for each enrolled child.
Evidence:
Child #4 did not have results of a health examination as required.

Description: Based on records review and interview, the provider did not obtain documentation of the current rabies vaccination.
Evidence:
Pet #2 did not have current rabies vaccination.

Plan of Correction: Rabies expired 8/3/2024. A vet 08/30/2024 appointment is scheduled. The rabies record will be updated on the same day.
