A complaint was received at the Central licensing office on 8/13/2015 regarding program and special care relating to infant care in the schoolage classroom. The complaint was initiated by phone on 8/18/15 by the licensing inspector and a complaint inspection was conducted on site on 9/2/15. Information had been provided by phone and during the inspection regarding the complaint. Based on observation and information provided, the preponderence of evidence supports the allegations. Therefore the complaint is determined to be valid.

Description: Based on information obtained during the complaint inspection, the facility did not provide a crib during rest time for infants. Evidence: On 8/13/15, eight infants and two staff were in the schoolage room for approximately 3 - 4 hours while the administrator cleaned the carpet in the infant room. The schoolage children were on a field trip. Cribs were not taken downstairs to the schoolage room and infants slept in bouncy seats, on boppy pillows and in a swing. Infant bottles were kept in the refrigerator and bottle warmers were brought down to the schoolage room.

Plan of Correction: We will clean the carpets either in the evening or on the weekend so children can remain in the infant room.

Description: Based on information obtained during the inspection, the facility used a table where children's activities or meals are served for changing diapers. Evidence: The changing pad from the infant room was placed on a table in the school age room and used for diapering. While the table was not used for activities or meals during the time the infants were present, it is used at times when schoolagers are present in the room. Licensing standards prohibit using a table that is used for children's activities or meals as a changing table.

Plan of Correction: We will use the changing tables in the infant room for diapering.
