22VAC40-185 STAFFING AND SUPERVISION.
An unannounced self-reported incident "other" inspection was conducted on this date. Interviews were conducted and documentation was reviewed regarding a child being found inside the classroom out of sight and sound supervision for approximately five minutes. The self-reported incident is determined to be "valid." Any areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-185-340-F Description: Based on self-report by the facility a child was out of sight and sound supervision for approximately five minutes. Evidence: 1. On 09/11/2015 staff #3 wrote the licensing inspector advising her of an incident where a toddler was out of sight and sound supervision for approximately five minutes that afternoon. 2. On 09/23/2015 the licensing inspector reviewed documentation of the incident and actions taken by the facility to prevent this from happening in the future. Interviews were conducted and information gathered indicating that child #1 was out of sight and sound supervision for approximately five minutes in the classroom while the other children were outside the afternoon of 09/11/2015. Plan of Correction: Staff did a head count that day before going outside and then again once outside. Apparently the child slipped back into the classroom. We have implemented a written checklist for name to face accountability when transitioning from inside the facility to the outdoor area. Also, we will continue counting heads but now we do so out loud.
