Comments:
Today's visit was a routine monitoring inspection. The inspection began at 9:30am and concluded at 2:00pm. During the inspection there were fourteen children in care with five staff members and additional support staff were also present. The following was either observed or reviewed during the time of the inspection: three out of fifty-five children's records were reviewed, two out of ten staff records were reviewed, free time in the gym, lunch time, emergency supplies, nap time, medication and some policies and procedures. Discussion took place regarding the position of today's Violation Notice at the downstairs entrance. This is a main entrance for morning drop-off and the main entrance for the school age children as the classroom is located on the lower level.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on the inspector's review of a sample of three out of fifty five children's records, the center failed to ensure a child had an immunization record before the child attended the center. Evidence: Child #2 has a start date of 2/16/15 and an immunization record dated 2/23/15. Immunizations or Certificate of Religious Exemption affidavit are required before a child can attend the center.

Plan of Correction: Individuals responsible for admissions will ensure each child has an immunization record before tha child can attend the center.

Standard #: 22VAC40-185-500-B
Description: Based on the inspector's observations in the downstairs rest room, the center failed to ensure soiled diapers were disposed of in a way that neither the staff member's hands nor the soiled diaper touches an exterior surface of the storage system. Evidence: The inspector observed a staff member change a diaper in the downstairs rest room. The soiled diaper was disposed of in a manner that is not hands free. The lid to the trash can swings open with the use of hands.

Plan of Correction: The Program Director will purchase a trash can that is operable by foot to be placed in the rest room and staff will be instucted to use the trash can in a hands free manner only when disposing of soiled diapers.

Standard #: 22VAC40-185-510-C
Description: Based on the inspector's review of two medications in the center's medication lock box, the center failed to ensure a medication authorization was available to staff for one out of two medications. . Evidence: The inspector observed an Epi-pen in the medication lock box for a child who is currently enrolled in the center. There was no written authorization available to staff in the event the Epi Pen would have to be administered. There was written information regarding the child's allergies that may trigger anaphylaxis.

Plan of Correction: The Program Director will provide the parent with a medication authorization form by the end of the program day and request that it is returned to the center as soon as possible, but no later than the end of the program week.

Standard #: 22VAC40-185-520-A
Description: Based on the inspector's observations, the center failed to ensure an over the counter skin product was not kept beyond the expiration date. Evidence: The inspector observed a tube of triple antibiotic ointment in the downstairs first aid kit with an expiration date of 3/15.

Plan of Correction: The ointment was disposed of during the time of the inspection. First aid kits will be monitored at least monthly.
