Today an unannounced monitoring visit was conducted with the Director-Mrs. Alison. This visit took place in the morning between 10:15am-1:30pm. There were 5 classrooms observed with a total of 97 students with 16 staff within the supervision guidelines. The children were observed having morning snack, circle time, learning to trace/write the letters, music class, outdoor play, lunch and in transition for nap time. Lunch today consisted of teriyaki chicken, carrots, rice, pears and milk. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, medication and emergency procedures were observed during this inspection. Standard #: 22VAC40-185-280-B Description: Based on observation of the physical plant, cleaning agents were found in the classrooms unlocked and accessible to children. Evidence: In the main hallway outside of Nemo's world a closet was found with the safety lock not secured with cleaning agents accessible. In Dora's Explorer's classroom, cleaning material were found under the sink unlocked and accessible. Plan of Correction: Director stated that teachers will be reminded to keep all cabinets and closets locked at all times.
Standard #: 22VAC40-185-280-H Description: Based on observation of the classroom, a purse was found in a classroom on a shelf accessible to children and unlocked. Evidence: In Dora's Explorer classroom, a purse was found on a counter near the window accessible to children. Plan of Correction: Staff will be reminded to keep all purses in locked cabinets at all times.
Standard #: 22VAC40-185-340-D Description: Based on observation of the groupings of children, in one classroom a program leader was found supervising more than two teacher aides. Evidence: In the pooh's pal classroom, the lead teacher was found supervising three teacher aides. Plan of Correction: Director stated that one of the aides meet the qualifications however she will have them bring a copy of her degree by tomorrow.
Standard #: 22VAC40-185-510-D Description: Based on observation of the medication found on-site, two medications were found without an authorization form. Evidence: Two emergency medications were found on-site without medication authorization form. Plan of Correction: Director stated that the medication and forms will be sent home today and parents will be reminded that the medication can not be returned until signed by a physician.
Standard #: 22VAC40-185-560-F Description: Based on observation of the classrooms and food provided, the center did not have a snack menu posted listing the foods being served for the current one-week period. Evidence: In 4 out of 5 of the classrooms, a snack menu listing the foods to be served for a current one-week period were posted however they did not include the date. Plan of Correction: Director stated that teachers will add a post-it note to the menu's to include the dates.
Standard #: 22VAC40-185-560-G Description: Based on observation of the classrooms, two lunch containers brought from home were found missing a date. Evidence: In the two out of 5 of the classrooms, two lunch containers brought from home were found missing today date and or name. Plan of Correction: Director will remind teachers to add a name and date on all lunch bags each day.
