Technical Assistance:
Consultation was given regarding announcement that effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. Comments:
An unannounced, on-site renewal inspection was conducted on 9/24/2024, as a part of the licensure period. The on-site inspection began at 9:55am and ended at 11:00am. The inspector reviewed compliance in the areas listed above. There were 9 children present and 3 caregivers; the point total was 18. The inspector reviewed 3 children?s records, 2 staff records and 1 household member record on-site. This inspection included document review of policies and procedures, emergency drill logs, medication administration logs, emergency supplies, authorization forms,tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-60-B
Description: Children's records are required to written authorization if a caregiver is to administer prescription or nonprescription medication.
Children's records are required to contain name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached.

The record for child #2 was missing parental authorization to administer prescription medications that were on site.

The record for child #1 was missing name, address and telephone number of two designated persons to contact in case of emergency and the parents cannot be reached.

Plan of Correction: Provider will request information from parent today. Provider will review Child record prior to first day of attending to ensure all information is provided.

Standard #: 8VAC20-800-70-B
Description: Before the child's first day of attendance, the provider shall obtain the parent's written acknowledgement of the receipt of the information regarding family day home operations and administration.

There was not parent acknowledgement available for child #1 who has been in care for two months.

Plan of Correction: Provider will have parent review and sign agreement form today. Provider will review children's files prior to/on the first day of attending to ensure completion.

Standard #: 8VAC20-800-90-A
Description: Before attendance, documentation of immunizations is to be obtained for each child.

There was no documentation of immunizations for child #3, who has been in care for over a week.

Plan of Correction: Provider will request information from parent today. Provider will ensure documentation required prior to first day of attendance is collected, prior to first day of attendance.

Standard #: 8VAC20-800-90-C
Description: Documentation of additional immunizations is required once every six months for children under the age of two years. 'Child #2, 15 months old, last documented immunization was over 9 months ago.

Plan of Correction: Provider will request updated record today. Provider will review children's records periodically to ensure up to date.

Standard #: 8VAC20-800-210-A
Description: Caregivers shall obtain a minimum of 16 clock hours of training annually.
Caregiver #2 did not have documentation of trainings in the past 12 months.

Plan of Correction: Provider will have caregiver provide certificates of completing 16 hours of training. Provider will collect certificates and record training hours routinely.
