Comments:
An unannounced renewal inspection was conducted from 9:21 a.m. - 1:30 p.m. There were seventy-nine children in care with fifteen teachers working directly with them. Four children's records were reviewed. Two staff records were reviewed. The infants were observed being fed, diapered, scooting, crawling, standing up with help, babbling and being talked with by staff, being read a story, having their backs rubbed, being fed, rocked and while sleeping. The older children were observed during hand washing, using inside voices and walking feet, during snack and lunch, engaged in story time and discussion, identifying colors, counting, discussing items at the nature table, working on making a porcupine from items found in nature, making stamp art, playing with toys, looking at books, outside playing, using their manners during lunch and asleep at nap. Description: Based on documentation and interview the facility failed to have documentation of an annual fire inspection. Evidence: 1. On 10/13/2015 the licensing inspector request to review the most recent annual fire inspection which was dated 09/24/2014. 2. On 10/13/2015 staff #1 confirmed the last annual fire inspection was 09/24/2014.

Plan of Correction: We have requested the fire inspection and it is scheduled to take place Thursday. We will scan a copy of our inspection to the licensing inspector for the record.

Standard #: 22VAC40-185-260-B
Description: Based on documentation and interview the facility failed to have documentation of an annual approval from the health department. Evidence: 1. On 10/13/2015 the licensing inspector requested to review the annual approval from the health department. The date of the last health inspection was 09/15/2014. 2. On 10/13/2015 staff #1 confirmed that the last health inspection had been 09/15/2014.

Plan of Correction: A health inspection was immediately requested. We will scan this inspection to the licensing inspector for the record.
