22VAC40-191 Background Checks (22VAC40-191)

Technical Assistance:
Discussed training requirements for staff and offered a sample of how to review and assess a year's worth of injury reports. Immunization updates were also discussed.

An unannounced renewal inspection was conducted from 10 - 12. The following ratios were observed : Infants 7 to 1, Toddler class had 5 to 2, Twos were 6 to 1, and preschool was 8 to 1. As the Inspector was reviewing paper work and records, more children did arrive and greeting of families was observed. The Inspector also reviewed activities with the children and interactions of staff with the children. Records, updated clearances, menu, emergency plans, drills, and emergency supplies were reviewed. Standard #: 22VAC40-185-160-A
Description: Based on record review the center has not obtained documentation of a negative tuberculosis screening within 21 days of employment. Evidence: 1. Staff 1 has been employed since August, 2015 and the documentation of a TB Screening is not in the file.

Plan of Correction: We have requested the screening and will request again.

Standard #: 22VAC40-185-40-J
Description: Based on staff interviews, the center has not updated the injury prevention procedures. Evidence: 1. Inspector asked for the Injury Prevention plan to see if it has been updated and the Director stated she had recently learned that was required but needed to complete.

Plan of Correction: We will review accidents and make written recommendations to complete the procedures.

Standard #: 22VAC40-185-70-A
Description: Based on record review the center has not obtained written information to demonstrate that the individual possesses the education component for the lead position. Evidence: 1. Staff #1 was hired August 10, 2015 as a lead. File contains verification of experience required for the position but the file does not have school transcript.

Plan of Correction: We requested the documentation during the visit and will place in the file.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the areas of the center, inside, are not maintained in a clean condition. Evidence: 1. Wooden floor molding around the center has dirt and caked on substance and this also includes the flooring directly attached to that molding.

Plan of Correction: We have put in an order for the floors and areas around to be professionally cleaned.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center has failed to keep hazardous substances such as cleaning materials, insecticides, and pesticides in a locked place using a safe locking method that prevents access by children. Evidence: 1. The center has a maintenance closet that has two entrance locations (one in the school room and one in Preschool room). Both doors leading into the closet were unlocked. 2. Inside the closet, and close to both entrances, the Inspector observed contents of cleaning supplies and hazardous building materials.

Plan of Correction: We will talk and remind the staff that the doors must remained locked to prevent possible access by children.
