During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. Comments:
An unannounced, on-site monitoring inspection was completed on 10/9/2024. The on-site inspection began at 10:05 am and ended at 11:30 am. The inspector reviewed compliance in the areas listed above. There were 8 children present and 4 caregivers; the point total was 27. The inspector reviewed 4 children?s records and 4 caregiver/household members records.
This inspection included document review, tour of the facility, interviews, observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-960-E
Description: Bottles shall be refrigerated and labeled with the child's full name and the date.

About 6 bottles for 3 different infants were found on the kitchen counter upon the Inspectors arrival at 10:05am, the bottles did not have the infants full name and were not dated.

Plan of Correction: All bottles will be labeled with child's full name, dated and placed in the refrigerator.

Standard #: 8VAC20-800-60-B
Description: Children's records are required to contain name, number and address of 2 designated people to contact in case of an emergency, and written authorization for emergency medical care.
The record for child # 4 was missing name, number and address of 2 designated people to contact in case of an emergency, and written authorization for emergency medical care.

Plan of Correction: Parent will complete the missing information

Standard #: 8VAC20-800-70-A
Description: Before the child's first day of attendance, parents shall be provided required information in writing.
The parents of child # 3 was not provided in writing policies for the administration of medication and provisions of the family day home emergency preparedness and response plan.

Plan of Correction: Provider will provide the information to the parents for written approval and then file in the child's record.

Standard #: 8VAC20-800-170-A
Description: Providers must obtain a tuberculosis screening from a caregiver at the time of hire and prior to coming into contact with children.
There was no record of tuberculosis screening for caregiver # 4 who has been caring for children for 3 weeks.

Plan of Correction: Caregiver will get tuberculosis screening.
