Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:

Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. Comments:

An unannounced, on-site monitoring inspection was initiated and completed on October 15, 2024. The on-site inspection began at 9:00am and ended at 10:00am. The inspector reviewed compliance in the areas listed above. There were 11 children present and 3 caregivers; the point total was 34 The inspector reviewed 5 children?s records and 3 caregiver/household member(s) records on-site. This inspection included document review, tour of the facility and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:

Standard #:	8VAC20-800-780-B
Description:	Annually, the provider shall review with the parent the emergency contact information required in each child's record to ensure the information is correct and shall obtain the parents signed acknowledgment of the review. The records for children #1 and #2 did not have a parent's signature for an annual review of the emergency contact information. Child #1 has been in care for over 4 years without an annual review for over 9 months. Child # 2 has been in care for 2 years and has not had an annual review for 21 months.
Plan of Correction:	I will make sure all parents review their child's record and sign off on it.
Standard #:	8VAC20-800-180-A
Description:	A tuberculosis screening is required every two years from the last screening. An updated tuberculosis screen for caregiver #1 was not completed and was overdue by two weeks.
Plan of Correction:	The TB screening will be completed and results will be placed in their record.
