Comments:
An unannounced monitoring inspection was conducted on October 21, 2015. There were 45 children present with six staff members. Additional staff were on site for administrative, volunteer and support purposes. Activities observed in the three classrooms operating on this date included center time, circle time and outdoor play. The posted menu indicated that today's lunch consisted of beefy mac, tossed salad, diced peaches, breadsticks and milk. Three children's records and one medication was reviewed during the inspection.

Description: Based on observation and inspection of the facility, the licensee did not ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. Fall zones are defined as the area underneath and surrounding equipment that requires a resilient surface. A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use. Evidence: 1. During the inspection conducted on October 21, 2015, there was an inadequate amount of resilient surfacing (pea gravel) in the fall zone at the end of the red slide chute. There was an approximate depth of two to four inches of surfacing at the base of the slide and in some areas of the fall zone surrounding the equipment where there should have been at least six inches. 2. There was an inadequate amount of surfacing surrounding one of two curvy ladders on the playground. There was approximately three to four inches of surfacing in some areas of the fall zone.

Plan of Correction: During the inspection, the teachers made the equipment "off limits" to the children in care. A work order will be placed with maintenance to have an adequate supply of pea gravel put down.
