Technical Assistance:
Consultation was given on infant pacifers that have small stuffed animals attached (WubbaNub) and ensuring that they are not placed in cribs when infants sleep. Please refer to CDC Technical Assistance for infant care and reducing risk of suffocation.

Comments:
An unannounced monitoring visit was conducted. All 5 rooms were in ratio with direct care staff. Observed infant care, bottle and food storage, lunch, activities for preschool children and rest time. Lunch served today was mac and cheese, broccoli and orange slices. Reviewed four children's files, medication on-site, trainings (MAT/First Aid and CPR certification) as well as emergency preparation. The playground was not reviewed due to heavy rain. Mulch was delivered in September as reported by the center director. Staff have been consistent for many years. All required postings are up to date. Standard #: 22VAC40-185-270-A
Description: Based on observation, two areas are not being maintained in a clean manner. Evidence: 1. The paint is chipped in several areas in the preschool room (room 4) along the window sills. 2. The rubber edging is torn in several places in the two's room along the window sill.

Plan of Correction: Paint touch up and rubber edging replacement complted.
