An unannounced inspection has been conducted from 4:05-5:10 pm. There were 54 children with 6 staff and the kids were in various activities, and located in different parts of the school. The Inspector observed the program, ratios, transition, and reviewed staff as well as children's records. The Inspector also observed children and staff interaction with art projects and homework. Standard #: 22VAC40-185-60-A Description: Based on record review of five children's files, the center has not maintained children's records. Evidence: 1. Child # 3 have not been annually updated and signed by the parent to verify information is still current. 2. Child #1's file and #2's file do not contain physical examination reports. 3. Child #2 has left program but file does not indicate termination date. Plan of Correction: We will request from the parents.
