- Technical Assistance:
1. Recommend the lock be removed from the inside of the bathroom door to prevent children from locking themselves inside. A key to the lock should be kept nearby in the mean time. 2. All sections of the written injury report are to be completed.
Conducted a unannounced monitoring inspection. Observed 10 children + 1 direct-care staff. Ratios and supervision were in compliance. The Lead Teacher today is also currently filling the role of Program Director. Children were observed reading books, playing with blocks, doing a counting activity, washing hands, eating snack, and going outside for a nature walk. The staff person was actively engaged with the children, providing guidance as needed, and praise for good manners. The staff demonstrated patience and kindness to a child needing to change their clothing. Snack served included: strawberries, crackers, apple juice or water. The classroom was found to be very clean, safeguarded appropriately, and well supplied with toys and equipment for the children. No medications reported to be on-site. Required documents were posted. Standard #: 22VAC40-185-280-B Description: Based on observations made, it was determined that a pesticide accessible to the children. Evidence: A bug glue type trap was on the floor by the preschool room door in the rear exit hallway. Children were observed leaving this door and passing by the bug trap as they went outside for play time. Plan of Correction: Bug trap was removed. Maintenance knows now not to place them by our door.
Standard #: 22VAC40-185-550-M Description: Based on records reviewed, it was determined that 2 of 4 written injury reports did not include all components of documentation needed. Evidence: One report did not include the time or how the parent was notified; another report did not include future actions to prevent recurrence. Plan of Correction: Injury records were completed with correct time, notification method, and future actions.
