Today we discussed medication, areas and equipment and hand washing.

An unannounced monitoring inspection of the facility inside and outside and records was conducted from 9:45am ? 12:30pm. There were 88 children present with 12 teaching staff and two family advocate workers. Twelve children?s records were reviewed. Emergency contact information was reviewed for 22 staff which included those present and those that may be present at any time. Interviews were conducted with children and staff present. Staff records are stored at a central location and will be reviewed at a later date. Children were observed in teacher and self-directed activities including outside play, transitions, restroom routines, breakfast and lunch, story time, center play activities and nap. The inspector reviewed current insurance documentation, fire and health inspections and emergency evacuation drills. The outside play space was observed. Nine medications and authorizations were reviewed. With parent permission some children are transported to and from the center daily. Parent authorizations were reviewed during record review. The following violations were observed and are cited on the violation notice, areas & equipment, medication, children?s records, hand washing, food preparation.qui

Standard #: 22VAC40-185-60-A
Description: Based on observation and interview the center failed to ensure that each child's record contains all required information. Evidence: 1 - In classroom D the records for children 1 and 2 do not include first date of attendance. 2 - Child 2 was present for care during the inspection. Child 1 was not present today. However, staff 3 reports that child 1 does regularly attend the child care program.

Plan of Correction: Center Coordinator will remind and retrain family advocates of the importance of including the first date of attendance on each child's application.

Standard #: 22VAC40-185-270-A
Description: Based on observation the center failed to ensure that areas are maintained in safe condition. Evidence: 1 - In classroom E there is a radio/CD plugged into the wall. The plug is placed across a portion of a nearby, child size, couch and plugged into the back of the radio/CD player. This poses a entanglement hazard. 2 - In classroom F there is a walk-in closet where food and food related items are stored. A hot water heater has been added to this closet. However, the closet does not have a child proof latch or lock. Therefore, the hot water heater is accessible to children in care.

Plan of Correction: The plug was corrected during the inspection. Center Coordinator will contact upper management to have a child proof latch placed on the closet door.

Standard #: 22VAC40-185-500-A
Description: Based on observation and interview the center failed to ensure that proper hand washing was completed by adults and children before eating. Evidence: 1 - In classroom E children were observed sitting on the rug listening to a story. Staff 1 stopped reading and instructed the children to go to the tables for breakfast. 2 - When asked staff 1 stated that the children had washed their hands prior to hearing the story. 3 - Children were observed touching the rug, their hair and each other while sitting on the rug listening to the story.

Plan of Correction: Center Coordinator will remind staff of the necessity for hand washing just before children sit down to eat.

Description: Based on observation and interview the center failed to ensure that medication is labeled with the child's name. Evidence: 1 - In classroom F there is one inhaler and 1 Epi Pen Twin pack, that are not labeled with the child's name. 2 - Staff 2 confirmed that the child's name was on the medication.

Plan of Correction: Center Coordinator will retrain staff on requirements and policy regarding children's names on medication.

Standard #: 22VAC40-185-560-I
Description: Based on observation the failed to ensure that food is prepared in sanitary manner. Evidence: 1 - In classroom E staff 1 was observed handing out cups to each child, for milk, prior to children eating breakfast. Staff one ran out cups. Walked to the trash can removed the lid and discarded the wrapper that previously contained the cups. Staff was then handed a new package of cups which she opened and handed out to the remaining children.

Plan of Correction: Center Coordinator will remind staff of proper hand washing procedures.
