Comments:
An unannounced monitoring inspection conducted on October 22, 2015 from approximately 3:45 pm- 4:30pm. There were approximately 16 children present with 3 staff members. The children were observed eating snack, activity stations, bathroom procedures and outdoor time. Two children records were reviewed. Exit interview was conducted with the Site Director.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on record review and staff interview, the licensee did not ensure each staff at the center had a name, address and telephone number of a person to be notified in an emergency. Evidence: During the inspection conducted on October 22, 2015, there was no emergency contact information for Staff #1 and Staff #2 at the center.

Plan of Correction: We will have an up to date emergency contact information sheet for all staff.

Standard #: 22VAC40-185-260-A
Description: Based on record review, the licensee did not ensure the fire inspection was done annually. Evidence: During the inspection conducted on October 22, 2015, the last fire inspection was conducted on October 9, 2014.

Plan of Correction: When the fire inspection is done next month we will have and up to form on file.
